To maintain confidentiality, DYSmd™ suggests that you choose a username and password different from your real name. You are responsible for keeping your password secret. Any individual with your username and password can access your account and all information stored within that account. Therefore, to ensure that unauthorized parties cannot access the personal chart you have created containing your medical history and other personal information (The Complete Medical Chart), you should never share your username and password with others.

E-mail Address

Although DYSmd™ tries to keep direct communication with you as limited as possible, it asks for your e-mail address to contact you regarding updates and services, to assist you if you have forgotten your username and/or password and to notify you regarding an upcoming expiration of your subscription. Additionally, you can choose to receive reminders about doctor’s appointments or prescription refills, which you have scheduled using your personal DYSmd™ Calendar. Please be aware that DYSmd™ is not responsible for any personal information you provide in an e-mail. Since an e-mail address is considered personally identifiable information, DYSmd™ encourages you to take advantage of the variety of free e-mail providers to set up an anonymous e-mail account and keep your current e-mail address private.

Date of Birth and Gender

DYSmd™ asks you to enter your date of birth and gender. Health conditions and medical treatments are different throughout age groups and vary for males and females. DYSmd™ uses your age and gender to offer you medical research, risk factor analysis and medical guidelines which relate specifically to your individual needs. Both age and gender are considered non-personal details when not connected with other personally identifiable information, such as name, address, e-mail address and/or telephone number. However, even with the knowledge of your e-mail address, your date of birth and gender will not reveal your identity to DYSmd™.

Health Information

In order to supply DYSmd™ users with medical information that is personalized to his or her health situation, DYSmd™ uses personal details. These details relate to past and current health conditions, medical treatments and family health history that are stored in The Complete Medical Chart. The more DYSmd™ knows about these factors, the more individualized health information and other benefits DYSmd.com can offer. All details related to your health are securely stored on DYSmd.com and are only available with a correct user name and password.

For Parents and Guardians

DYSmd.com is not intended for children and adolescents younger than 18 years of age. Additionally, you must be 18 years of age or older to be eligible to establish an account with DYSmd.com. However, a parent or guardian may choose to include a minor (under the age of 18 years) in the DYSmd™ Family Plan. In this case, the parent or guardian is considered solely accountable for keeping the provided personal information of that minor secure. Additionally, it is solely the parent's or guardian's responsibility as to how the registered minor uses and interprets services and suggestions provided by DYSmd.com.

By making use of DYSmd.com, its content and services you are agreeing to the Terms and Conditions of Use, assuring you are of sufficient age to create an account, and agreeing to the Terms and Conditions of Use for any minor or other persons included in your DYSmd™ Family Plan.

Changing Personal Information or Canceling an Account

At any time, you may change provided personal information or cancel your account by clicking on Account at the top of DYSmd.com. Please note that the personal data you have supplied will be stored in DYSmd™'s secure system for 1 year after the cancellation or expiration of your account. This is to avoid the loss of all subscription information in the event of an unintentional expiration or in case you change your mind and would like to re-subscribe to DYSmd™ within this time period.

Third Parties

Third parties may pay DYSmd™ to make information concerning their health or medical products available to you. Advertising related to your specific health condition may be displaying while you are logged on to DYSmd.com. However, since it is not possible for DYSmd™ to make an association between a user's account and his or her Complete Medical Chart, your personal information (e.g., e-mail address, date of birth) will not be shared with these advertising companies. DYSmd™ may also use your presented information to offer links to web sites which are thought to be of interest to DYSmd™ users. DYSmd™ does not take responsibility for the privacy policies and the usage of cookies (see description below) of such other sites. Pay Pal

The personal and payment information collected by PayPal comes within the scope of PayPal's own privacy policy. DYSmd™ is not responsible for the personal information you provide to PayPal. For the exact details concerning the use of your private information, DYSmd™ suggests reviewing PayPal's privacy policy.

Furthermore, the private information you provide to PayPal will remain unknown to DYSmd.com. There is no possibility of DYSmd™ gaining access to the information supplied to PayPal or to connect such information to your Complete Medical Chart. Updates regarding a customer's payment status are automatically sent by PayPal to the individual DYSmd™ account. Outstanding payments will result in blocked access to your account until proper payment is received, without DYSmd™ staff being involved in such actions.

Aggregate Data

DYSmd™ may use certain non-personal details that a user has provided to develop business statistics that will help improve its services. These non-identifying details form the so-called "aggregate data". The purpose of gathering aggregate data is to summarize the patterns and features of a large group of anonymous people.
